Notes on how to complete form

  1. Each individual senior, junior or veterans player must complete a separate form for insurance coverage with both the club and respective cricket associations.

  2. If applying for family membership, then non-playing members need to be included on page 2 of this form. If there are two or more playing members from the one family membership, the family details need to be only listed once.

  3. Junior players must have their form signed by parent or guardian.

Note 1: Single memberships are included/factored into the above fees.

Note 2: Junior daily match fees are included in the above fees.

Note 3: A student playing member is one who is attending secondary school, is completing a trade apprenticeship, or is deemed a full-time university student (but can be working part-time) and is not a junior member.

Note 4: A junior playing member is one who is attending Milo In2Cricket or playing under 10s, under 12s, under 14s, under 16s or under 18s.

Note 5: If more than 2 members from the same family are paid up, then additional non-playing family members can become club members at no additional cost.

Note 6: Veterans can select to pay membership and annual fees, or $20 match fees.

Note 7: All players selected (and having indicated they are available for selection) must pay the annual membership fee of $50 as part of the annual subscription.

Note 8: Any player selected in less than 50% of the games during a season will be liable to pay 50% of the annual subscription (this does not apply to student< JDS seniors or veterans players).

Note 9: Any player selected in 50% or more of the games during a season will be liable for the full subscription.

The fee for the senior game is the difference between the junior fee and the senior fee. For example, if a player plays in U16s on a Saturday morning ($0) and then again in a senior game (non-turf) on the afternoon of the same day, then payment will be $2 for the afternoon game. If the same situation occurred but the junior played in a turf side on the Saturday afternoon, then $0 would be paid in the morning and then another $5 for the afternoon game.

How to Pay

The following payment options exist.

EFT – Electronic Funds Transfer

You can make an online transfer into our bank account. The club’s account details are as follows:

Please make sure you use your surname as reference (so that we can track payment through our statements) and keep details of receipt.

EFTPOS

Our EFTPOS machine will be available on registration day or at the clubrooms once the season starts. Please keep your receipt.

Cheque

Please make cheques payable to East Doncaster Cricket Club and mark as ‘not negotiable’. This can be given to a designated representative on registration day or to a member of the club executive at any other time.

Cash

If the above methods don’t suit, cash is accepted at registration day to a designated representative or to a member of the club executive at any other time. Please ask for a receipt.

Note 1:- A surcharge of $50 will apply if your membership is not paid by 1 November 2013. Please speak to a member of the club executive if assistance is required or you would like a payment plan established.

Doncare Community Partnership

Doncare has worked & supported the Manningham community tirelessly over the past 42 years, providing those within our community that are less fortunate than ourselves a helping hand through Social support, Family services, Emergency relief, Grief counselling, Christmas & food programs along with a Back to school program that provides uniforms & books to in excess of 150 kids in our community.

The East Doncaster Cricket Club has struck up a Community Agreement with Doncare to support this organisation and ensure they continue to support the local community, we would encourage .

Working with Children

From 1 July 2009, any volunteer involved in a sporting club that has direct contact with children must have completed a Working with Children (WWC) check.

WWCs can be completed at any Australia Post outlet. Please collect and complete the form (do not sign), collect 100 points of ID and a passport-size photo and present yourself at Australia Post to complete and sign the application.

All successful applicants for a WWC will receive a photo ID card and will be registered in My Cricket as a WWC holder. Valid WWCs last for 5 years from the date of issue.

All team managers, coaches, captains and club executive members must hold a current WWC.

Insurance

All registered playing members of the East Doncaster Cricket Club are covered by Cricket Australia’s insurance provider – JLT Insurance.

For details on insurance cover and on how to claim, please contact the club secretary on secretary@.au or refer to .au/cricketaustralia/ for detailed information on what is covered.

Junior Parent Declaration (This declaration covers all children listed on this form)

I consent to my child participating with the East Doncaster Cricket Club (EDCC) in relation to all training, matches, competitions and other cricket related events and programs for the 2013-14 season. I acknowledge that cricket can be inherently dangerous and that serious accidents can happen, which may result in my child being injured.

Except to the extent that the Trade Practices Act 1974 (Cth) or other legislation applies, and cannot by contract be excluded, I agree, both on behalf of my child and in my own right, that it is a term of my child’s participation that EDCC is absolved and indemnified from all liability however arising from injury or damage however caused arising out of my child’s participation in the 2013-14 cricket season with EDCC or in any way due to any negligent act, breach of duty, default and/or omission on the part of EDCC.

I agree, both on behalf of my child and in my own right, to release and forever discharge the EDCC from all claims (except those claims made and accepted under a relevant EDCC insurance policy, to the extent of the cover provided under the policy – summary available upon request) that I or my child may have or may have had but for this release arising from or in connection with my child’s participation in EDCC’s training, matches, program, competitions or other cricketing related events.

I confirm that I am a parent or legal guardian of the child(ren) and that I am responsible for organising transportation of the child to and from cricket activities. I indemnify the EDCC for all liability and costs associated with my failure to arrange for the prompt collection of my child after the completion of cricket activities.

I authorise EDCC representatives to arrange medical or hospital treatment (including ambulance transportation) if I am not available to do so and I indemnify the EDCC of all costs associated therewith.

Privacy Statement: EDCC is committed to the protection of your personal information. Any personal information you provide to the EDCC will be used for those purposes, which the information was gathered for as stated and related purposes which can be reasonably expected.
